Coordinator - Infection Prevention and Quality

Assists Hospital leadership in the planning, implementation and evaluation of the infection prevention, clinical quality and patient safety initiatives in compliance with accreditation standards.
Performs a variety of surveillance, administrative and educational activities regarding infection prevention, quality and patient safety.
Ensures compliance with infection control policies and procedures and external legal regulations.
Identifies infection prevention problems, documents and recommends corrective actions.
Serves as a project facilitator for entity infection prevention, quality and patient safety, while also serving as a local change agent for the hospital and other areas as assigned.
Collects data for reporting requirements and recommending improvement activities for infection prevention, quality and patient safety within the organization.

Assists Hospital leadership in the planning, implementation and evaluation of infection prevention, clinical quality and patient safety initiatives by:
Working collaboratively with Hospital leadership and care teams to monitor adherence with patient safety standards.
Establishing and maintaining effective communications and working relationships with key personnel and hospital departments.
Monitoring and alerting Hospital leadership for trends in infection prevention, clinical outcomes and patient safety events.
Supporting Hospital and Beacon system committees requiring quality outcome data information.
Coordinating or initiating correspondence regarding quality plans to payers and other outside agencies.
Enhances infection prevention, clinical quality and patient safety by:
Understanding, disseminating and communicating trends and gaps in quality and safety data and outcomes.
Assisting with patient safety events, trends and action plans as it relates to infection prevention, clinical quality and patient safety.
Responding to serious safety events and communicating with leadership as appropriate.
Communicating and providing feedback on infection prevention policies and procedures, identification and isolation of infections, and issues to all departments involving physicians, administration, nursing staff, the Infection Control Committee, and managers.
Assisting with the implementation and adherence of all clinical/safety related policies to foster a patient safety culture throughout the hospital.
Reviewing and maintaining departmental policies and procedures relating to infection prevention, quality and patient safety.
Promoting a culture of safety by assisting with the Patient Safety Culture Survey and action plan.
Making rounds to areas under the hospital license and assisting with Infection Prevention concerns and implementation of infection prevention practices.
Collecting and preparing agenda and reports for the Infection Control Committee; making recommendations and carrying out re-evaluation of problems identified, as indicated.
Referring employees exposed to communicable/reportable diseases to Employee Health.
Supporting compliance of external and internal regulatory requirements related to accreditation and licensure including infection prevention.
Providing education for staff and providers as indicated.
Collects data for reporting requirements and recommending improvement activities within the organization by:
Analyzing data related to key performance standards identified by internal and external key stakeholders.
Completing, monitoring and reporting a variety of records, reports and statistical summaries as required or requested for leadership and with third party pay for performance programs.
Collaborating with multidisciplinary teams to identify gaps in quality measures and to implement solutions with the support of data.
Performs other functions to maintain personal competence and contribute to the overall effectiveness of the department by:
Acting as a resource to other department members to address day to day issues relating to infection prevention, quality outcomes and patient safety.
Participating in Hospital committees as designated.
Completing other job-related assignments and special projects as directed.

Education and Experience The knowledge, skills and abilities ad indicated below are normally acquire through the successful completion of a Bachelor's Degree in a healthcare related field.
Certified in Infection Control (CIC) certification required or will obtain within 3 years of hire.
A minimum of 2 to 5 years of job-related experience is required.
RN is strongly preferred.
Prefer knowledge of national standards and outcomes for quality, safety and healthcare - Anthem Measures and Value Based Purchasing, AHRQ, Leapfrog, as well as knowledge and experience in quality improvement strategies, such as Lean/Six Sigma, the IHI Model for Improvement, or other comparable performance improvement strategies.
Knowledge & Skills Requires well-developed knowledge of clinical microbiology, general principles of asepsis, and transmission methods of infectious disease, epidemiological process, and CDC Infection Control Guidelines.
Requires proficiency with basic computer skills to navigate through electronic medical records and current surveillance system.
Demonstrates analytical skills necessary to evaluate data and recommend changes in processes and procedures.
Demonstrates interpersonal skills necessary to interact effectively with a variety of contacts within and outside the Hospital (i.
e.
, medical staff, hospital department directors, external regulating agencies, etc.
).
Demonstrates well-developed communication skills (both verbal and written).
Demonstrates organizational and analytical skills necessary to independently work on patient and environmental safety issues and seeks consultation of other associates when appropriate.
Requires the ability to prioritize and to handle a multitude of assignments at one time.
Requires the ability to maintain strict confidentiality regarding all information utilized.
